Guidelines Title: Scanning of Students’ Multiple Choice Questionnaires/Exams/Tests.
Objective:
To provide a useful and efficient service for Lecturers who are marking and processing the Multiple Choice Questionnaires/Exams/Tests.
DEFINITION OF TERMS
- MCQ/Exams/Tests – Abbreviation for Multiple Choice Questionnaires/Exams/Tests.
- A set of questions the students are asked which can be answered by selecting one or more answers from a maximum choice of 5 possibilities.
- GPAS – General Purpose Answer Sheet (See Appendix A)
- Scanning – The process of reading information from a scannable form by an optical reader.
- Processing – Scanning, analysing and reporting of information scanned.
- Answer Key – A GPAS containing the correct answers for a particular MCQ/Exam/Test.
- Groups/Batches/Sets – A classification (eg: Tutorial Group; Afternoon Class) of papers scanned which may need to be analysed and reported separately
- MCQ_ChargeSheet.xls – A worksheet requesting/containing information about the lecturer, the scope of the job and the cost of the job. (See Appendix B)
- Survey Services – A section within Governance, Policy and Planning Services Centre (GPS).
Policy:
- MCQ/Exams/Tests to be scanned and processed by Survey Services using Scan Tools for Windows (Version II) software and Opscan 8 Optical Readers/Scanners.
- We do not accept internally received exam papers. Exam papers should be sent through courier/ registered mail or delivered personally.
- Bookings are essential.
- Exam papers delivered without bookings will be processed as a low priority.

What to do:
- Prior to Making a Booking:
1. The content of the MCQ/Exam/Test and answer key must be prepared prior to the booking of the scanning.
How to:
- Make a Booking for Processing:
1. Contact Survey Services and arrange a suitable time for the processing of the MCQ/Exam/Test. (If MCQ/Exam/Test is being collected then please contact Survey Services to arrange a suitable time for collection.) Survey Services is located on the Joondalup Campus, Building 1A. Room 1.390.

What to instruct students and invigilators to do:
- Prior to students taking the MCQ/Exam/Test:
1. Inform and instruct students how to fill in the GPAS and ensuring that their responses and identification details are completed/ filled in correctly (Identification details must be left justified. Student Id must be filled in Columns A – G of the Identification Number grid.) This is usually done 2 weeks leading to the MCQ/Exam/Test week.
2. Also remind students to have an eraser and a suitable pencil if they are answering in pencil otherwise a blue or black pen and White out if they are answering in Pen.
- On the day of the MCQ/Exam/Test:
1. Ask the invigilators to remind the students that their identification details are completed/ filled in correctly (Identification details must be left justified. Student Id must be filled in Columns A – G of the Identification Number grid.) and responses must be clearly and correctly marked on the GPAS.
2. Ask the invigilators when collecting the Exam Booklets and GPAS to ensure that all students have correctly filled in their Student Id’s and full name on the GPAS.
1. Collect all material from Student Services (the invigilators will collate and return all Exam/Test material to Student Services) and separate the GPAS from the rest of the Exam/Test material. Have all GPAS sorted and collated one way, ie: all same way facing up.
What to do/have ready for the Processing Appointment:
1. Have all GPAS sorted, grouped (if applicable) and labelled. Each group must be labelled (ie: Unit Code, Campus, etc…) and the answers must be filled out on the answer key. The answer key sent to Survey Services must be original not photocopy or faxed.
2. Upon arrival for Processing appointment with Survey Services, staff must complete the MCQ_ChargeSheet.xls and fill in the Authorised Cost Centre.
3. Advise Survey Services the types of reports they wish for including Electronic copies.
What do Academic Staff receive after the Processing is complete:
1. All GPAS and Answer key will be handed back to the staff with the reports in hard copy.
2. In addition to Electronic HTML copy of Results, a spreadsheet form, containing students Id No, Name, Score, will be provided upon request.
