Medical errors due to information failure
Medical error is defined a preventable adverse effect of healthcare on a patient. Medical errors represent a serious public health problem in all countries. This project will review the literature on medical and clinical errors as a result of information failure. This failure this may be in terms of systemic failures or information flow among other causes. The project will attempt to quantify these errors and make recommendations for improvement.
