Information for health professionals

Interprofessional learning (IPL) provides an opportunity to change the way we think about educating future health workers, to step back and reconsider the traditional means of health care delivery. The aim is not just to generate a change in educational practices but a change in the culture of medicine and health care. IPL aims to produce health workers who are willing and able to deliver healthcare through collaborative practice.

The IpAC Program provides a holistic approach to chronic disease self-management and can provide an extension to the expertise provided by the Vario Wellness Clinic under GP referred Enhanced Primary Care (EPC) plans. Alternatively, clients can be referred directly to the IpAC Program by the health professional involved with, or providing, their care.

The importance of enhancing an individual’s ability to self manage their chronic disease, from prevention of risk factors or risky behaviour to the various stages of management of their care, is well recognised. The current health system is ill equipped to educate and support individuals to self manage. The IpAC Program can offer services to fill this gap in service provision at no extra cost to the client. The students and health professionals educate and support clients in the management of their chronic disease. At all times students are supported by a team of experienced clinical supervisors from a variety of professional disciplines.

How can I refer my patient to the IpAC Unit?

Following discussion with your patient to determine their interest in attending a care program, a referral letter will need to be provided to the patient for presentation when they attend their first appointment. It is the patients responsibility to make the initial appointment.

Appointments are available Monday to Friday from 9.00am with the final appointment at 2.30pm. Bookings can be made on (08) 6304 3581.
